Email forms let your visitors contact you directly from your web site. Create surveys, collect important information, and have the results sent to the alternate email address you have listed in your Yahoo! Account Information.
You have a few options to install forms to a page.

- Copy and paste the HTML code for Sample Email Form 1 or Sample Email Form 2 to your page. You may use the forms as they are or edit them to suit your needs.
